First sick notes

For a first sick note someone from the practice may need to speak to you either over the phone or face to face.  Please call the practice on 0300 111 2300 to make an appointment. 

Repeat sick note

You can submit a request for a repeat sick note through our online consultation service -

A member of the team will review your online consultation and it will be dealt with in order of clinical priority.  In some cases clinicians are able to diagnose/advise using a combination of the online consultation and your clinical record meaning you may not need to be seen in person.
